Healthcare Privacy Policy

The privacy of your personal information is important to me. I am committed to collecting, using, and disclosing personal information responsibly and only to the extent necessary for the services I provide.

Like all medical professionals, I collect and use personal information to serve my patients.  The primary purpose for collecting personal information is to provide the best possible treatment by understanding what limitations may be present in regards to the type of massage you can receive based on your health condition(s), to help develop realistic treatment goals, and to assess the progress of our work.

Like most organizations, I also collect and use information for secondary purposes. The most common example of related and secondary purposes is to process credit card payments.

I retain patient information for a mandatory minimum of ten years after the last contact.

Protecting Personal Health Information

I understand the importance of protecting personal information. For that reason, I have taken the following steps:

  • Paper information is kept in a secured, restricted area and/or destroyed after converting to electronic records.

  • Electronic hardware is supervised or kept in a secured, restricted area.

  • Paper information is transmitted through sealed, address envelopes.

  • Electronic information is transmitted either through a direct line or has identifiers removed or is encrypted (i.e. information requested by your health insurance provider).

Disclosing Personal Health Information

There are very few circumstances that would require sharing or disclosing your personal health information to outside individuals. Below are some examples of how your healthcare information
may be used or disclosed:

  • Your health information, including treatment records may be disclosed to another health care provider if it is necessary to refer you to someone for diagnosis, assessment, or treatment for your health condition. Release of this health information will only be made
    with your consent.

  • Your contact information and health record may be used to follow-up after care has been provided and / or to send telephone or email appointment reminders. If you are not available to receive a telephone appointment reminder, a message will be left.

  • Information about my services, care information you have requested, special offers, or greeting cards may be sent via email or mailed to your home. Please let me know if you have specific contact preferences or prefer not to be contacted.

Your healthcare information may be disclosed without your consent in the following circumstances:

  • In an emergency

  • When required by law to disclose information to the Illinois Department of Health and other public health, law enforcement, and regulatory authorities.

Your right to privacy will always be respected. Other than the uses described above, your health information will not ever be sold or provided to any other outside organization.

Reviewing Your Personal Health Information

You have the right to see and review what personal information I hold about you.

I can help you identify what records I might have about you. I will also try to help you understand any information you do not understand (e.g., short forms, technical language, etc.).
